Potter County Community Based Care Program

The Potter County Community Based Care Program is funded through the Behavioral Health Alliance of Rural Pennsylvania. This program facilitates collaboration and partnerships in Potter County to assess, refer and mitigate social determinants of health addressing barriers of food insecurity, housing instability and homelessness, employment, financial strain, childcare access and utilities.

Childcare Access and Affordability

NTCAC will provide Potter County families  with access to a variety of comprehensive preschool programming in the provision of education, health, and nutritional services.  NTCAC will also assist with getting eligible children on a waiting list for when the program is full.

Financial Strain and Utilities

NTCAC will provide Potter County families with a variety of comprehensive weatherization services to homeowner occupied dwellings.  NTCAC will also assist with applications for crisis assistance, energy assistance, and for LIURP, which is a resource for energy education and conservation.

Food Insecurity and Access to Food

NTCAC will provide assistance through Potter County food banks for emergency food boxes to ensure a complete and nutritious diet.

Housing Instability and Homelessness

NTCAC will provide Potter County residents with homeless prevention services, to include assistance with PHARE and the Emergency Solutions Grant.  Homeless assistance may be available to individuals who are literally homeless or at risk of being homeless.  The Emergency Solutions Grant does have a priority for Veterans.
